4 Hole Offset Plate

1274A 4-Hole Offset Plate is intended to be used in the construction of limb sockets with a 4-hole bolt pattern. The central hole is intended for use with pin lock socket suspension systems. Suitability of the device for the prosthesis and the patient must be evaluated by a healthcare professional. The device must be fitted and adjusted by a healthcare professional. Intended for single device use only, not to be reused on another device.

Indications For Use and Target Patient Population
• Lower limb loss, amputation, or deficiency
• No known contraindications

Thread Locker
1. Be sure that the surface the plate is attaching to is completely flat. Any imperfections may cause undue stress on the part and could cause binding of the mounting holes of the subsequent 4-Hole component screws.
2. Tighten mounting screws according to the 4-Hole plate manufacturers specifications.
3. Tighten the 4-Hole mounting screws of the component below the 1274A Offset Plate to 10Nm.
